Care & Caregiving: Intervention development and dissemination

Create research programs to support the development of implementable dementia care and caregiving interventions and determine how and why methods for dissemination and implementation work to scale up promising practices in dementia care across settings and across the disease severity spectrum.

  • Support one Stage I study developing and pilot testing training materials for community providers to administer dementia care interventions with fidelity.
  • Support one Stage III study testing training materials for community providers to administer dementia care interventions with fidelity.
  • Support two Stage I, Stage II, or Stage III studies testing the essential components and/or hypothesized mechanisms of behavior change of dementia care interventions.
  • Support two Stage I studies simplifying dementia care interventions to be more readily implementable with fidelity.
  • Support two studies of simplified dementia care interventions (e.g., technology-based interventions, behavioral economic interventions) in phased pilot Stage I through Stage IV pragmatic trials.
  • Support two Stage I, II, or III studies of dementia care interventions that seek to adapt interventions to engage relevant putative targets associated with specific characteristics of groups of individuals in diverse populations, based upon the hypothesized mechanisms of change of interventions.
  • Support one Stage V study aimed at determining the principles underlying the best strategies to increase the adoption of dementia care interventions that can be administered with fidelity and that have a strong evidence base for efficacy.

Summary of Key Accomplishments

In response to research gaps and opportunities identified during the 2017 National Research Summit on Care, Services, and Supports for Persons with Dementia and their Caregivers, NIA invested in two large efforts to rigorously assess the state of the science on the readiness of dementia care interventions for widespread dissemination and implementation. Completed in 2020 and 2021, the Agency for Healthcare Research and Quality systematic review of dementia care and caregiving interventions and the National Academies of Sciences, Engineering, and Medicine consensus study report, point to the need for additional support for systematic intervention development that considers from the beginning the constraints and opportunities for broad real-world dissemination. As of 2024, seven projects have been awarded in response to NIA's calls for research to systematically build and tests interventions with an eye on future dissemination.

Several ongoing initiatives also contribute toward meeting the success criteria for this milestone, including the AD/ADRD Roybal Centers, and the IMPACT Collaboratory. The latter promotes care and caregiving interventions in home health, assisted living, hospitals, rehabilitation centers, nursing homes and other real-world settings. The IMPACT Collaboratory and its associated resources are helping researchers generate real-world evidence on the effectiveness of new treatments and care practices for older adults, individuals with disabilities, and people living with dementia in nursing homes, which can help identify and inform best practices going forward.

The key accomplishments summary is current as of July 2024.
