Self-Referral and Clinician Referral Forms

The homepage of the Indiana Alzheimer Disease Center (IADC) provides one-page online referral forms that patients or their clinicians can use, respectively, as a first step in volunteering for research on Alzheimer’s disease and other dementias. IADC reports that development of the electronic community referral forms and tracking system has enabled it to better capture referrals from all sources. In a recent reporting period, 51 percent of 259 referrals from physicians were successfully enrolled.
