This Biomarker Disclosure Toolkit provides example pre-disclosure information sheets about dementia, mild cognitive impairment, and dementia. Additionally, for each of these categories, the toolkit includes disclosure guides for individuals with and without elevated beta-amyloid. The page also includes an example of an informed consent form and provides information on the importance of discussion before biomarker testing.
